Care Manager/Medical Social Worker
Peace Health - St. Joseph Medical Center

Position: ED Social Work Position, hours from 1800-0230, and a 0.7 FTE, Evening shift, every other weekend. This position consists of emergency psychiatric social work, and requires a MSW with a proficiency in the diagnosing, risk assessment, disposition planning, and hospitalization of the acute and chronically medically ill. Is also responsible for provision of medical social work and case management services in the ED.

Location: Peace Health - St. Joseph Medical Center is located in Bellingham, Washington.

Job Summary: The Care Manager is responsible for identifying medically and psychosocially complex patients and families who are likely to benefit from care management and meet high risk criteria and for the coordination of care for these patients. This includes: collaborating with individuals and their families using the care management process for assessing needs, developing plan of care, coordinating delivery of services, monitoring changes, and reassessing the person's needs on a regular bases; working collaboratively in a shared leadership model with the RN Care Coordinator; working collaboratively across departments within the acute care setting and with community agencies and providers across the continuum of care; facilitating the coordinated utilization of resources for maximization of health outcomes, patient/family satisfaction and financial outcomes.

Qualification Summary: Masters degree in social work (MSW) is required. A minimum of two years employment in a healthcare setting or community agency dealing with health and/or welfare issues. Licensed Independent Clinical Social Worker or Washington State Agency Affiliated Counselor License is required. Certification in Case Management highly desired.

Job Summary: Social Worker will provide clinical social work services to children with acute and chronic illness, their families, the staff of Seattle Children's Hospital and the community in a managed care environment. Assist in promotion of culturally appropriate family-centered care and wellness of patients and families by identifying, interpreting, and reducing psychosocial factors contributing to illness. Counsel and aid patients and their families to understand and accept medical recommendations and options. Assess and assist with non-medical problems interfering with effective patient care. Interview, coordinate, and plan programs and activities to meet the social and emotional needs of patients and/or patients' family. Provide crisis intervention and assist families in understanding the implications and complexities of the medical situation and its impact on lifestyle.

MSW - Case Management - Skagit Valley Hospital
Includes Weekends

Job Summary: Provides inpatient and emergency department clinical social work services to address personal and environmental limitations that predispose illness or interfere with obtaining maximum benefits from medical, psychiatric or chemical dependency care. Social work performs risk assessments to identify and report harm to children and vulnerable populations. Coordinates discharges to services that provide continuity of care through referrals to home health, hospice, skilled nursing facilities, rehabilitation, psychiatric/substance abuse services, support groups, and/or social service agencies. Facilitates utilization of appropriate healthcare resources. Works collaboratively with others on assigned nursing units, emergency department and community to provide safe, patient centered, effective and efficient care. Works closely with the utilization review team regarding financial issues, length of stay and progression of care plan. The position must be available to work Saturday and Sunday each week.

Education: Minimum 1 year of professional paid social work experience or graduate practicum placement in a medical or psychiatric setting. MSW from an accredited social work program.

Experience: Hospital based experience preferred. Demonstrated competence in psychosocial assessments, discharge planning and crisis intervention.

Other Minimum Qualifications: Ability to practice collaboratively, work in a fast paced environment & prioritize job responsibilities in order to respond to multiple & sometimes competing requests. Knowledge of Social Work Standards of practice techniques and their application to complex case work, group work & community problems. Knowledge & experience with:DSM IV & the application in mental health assessments; crisis management, conflict resolution & stress management. Proven ability to intervene across a broad range of family, individual & group dynamics. Knowledge of health care laws, ethics, regulations & policies, which govern medical & psychiatric social work. Knowledge in needs assessment, resource identification, acquisition, community resources including local, state & regional resources. Experience in methods application, casework, supervision & training. Demonstrated commitment to social work values & adherence to the NASW Code of Ethics & NASW Standards of Cultural Competence of in Social Work.

License/Certification: Washington state licensure as a Licensed Independent Social Worker or Agency Affiliated Counselor is required.
